Welcome to SSK Hospital. We are committed to providing compassionate and comprehensive care during your stay. Please read the following guidelines carefully to understand our admission process, hospital facilities, and patient services.
Patient Registration Instructions (OPD/IPD)
- All first-time patients are required to fill the registration form at the registration counter. This is a mandatory one-time process.
- A valid government identity proof (Aadhaar Card, PAN Card, Passport, or Voter ID) must be provided for verification.
- A unique Patient Registration Number (PRN) will be generated after verification, which remains permanently linked to the patient.
- The information provided during registration cannot be altered later without valid government documentation supporting the change.
- Once PRN is generated, repeat registration is not required for future OPD or IPD visits. If already registered during first indoor admission, the same PRN will be used for OPD, and vice versa.
- The process may take 10–15 minutes from the time registration begins.
- A PRN card will be issued, which must be carried during every hospital visit for smooth access to your records.
- Accurate registration ensures proper documentation for purposes like government correspondence, insurance and medical record continuity.
रुग्ण नोंदणीसाठी सूचना (बाह्यरुग्ण /आंतररुग्ण विभाग )
- प्रथमच येणाऱ्या रुग्णांनी नोंदणी काउंटरवर नोंदणी फॉर्म भरणे आवश्यक आहे.
ही प्रक्रिया फक्त एकदाच केली जाते. - वैध सरकारी ओळखपत्र (आधार कार्ड, पॅन कार्ड, पासपोर्ट, मतदान ओळखपत्र) तपासणीसाठी आवश्यक आहे.
- तपासणीनंतर रुग्ण नोंदणी क्रमांक (PRN) तयार केला जाईल, जो रुग्णाशी कायमस्वरूपी जोडलेला असेल.
- नोंदणी दरम्यान दिलेली माहिती वैध सरकारी पुराव्याशिवाय भविष्यात बदलता येणार नाही.
- एकदा PRN तयार झाल्यावर, पुढील OPD किंवा IPD भेटींसाठी पुन्हा नोंदणी करण्याची आवश्यकता नाही. रुग्णालयात भरती होताना नोंदणी झाली असेल, तर बाह्य रुग्ण विभाग (ओपीडी) साठी पुन्हा नव्याने नोंदणी करण्याची गरज नाही. तसेच, बाह्य रुग्ण विभागात (ओपीडी) प्रथम नोंदणी झाली असेल, तर रुग्णालयात भरती होताना (आंतर रुग्ण विभाग – आयपीडी) पुन्हा नोंदणी करण्याची गरज नाही.
- ही नोंदणी प्रक्रिया सुरू झाल्यानंतर सुमारे १०–१५ मिनिटे लागू शकतात.
- नोंदणीनंतर एक PRN कार्ड दिले जाईल, जे प्रत्येक वेळेस रुग्णालयात आणणे आवश्यक आहे.
- आपण दिलेल्या अचूक माहितीमुळे सरकारी नियामक यंत्रणेकडे नियमाने कराव्या लागणाऱ्या नोंदी, विमा आणि वैद्यकीय रेकॉर्ड यामध्ये सातत्य राहते आणि भविष्यातील अडचणी टाळता येतात.

Admission Procedure
Before Admission:
The patient must be physically present at the hospital at the time of admission.
Admission Note from your treating consultant is mandatory. If not available, please visit the Emergency Department to obtain one from the CMO.
Kindly carry:
- 1)Valid government-issued ID (Aadhar, PAN, Voter ID, Passport, etc.).
- 2)Past medical records and discharge summaries (if any).
- 3) Personal items such as a comb, hair oil, toothbrush, toothpaste, napkin, and towel.
- 4) Do not carry jewellery, cash or valuables.
On Arrival:
You will be required to fill the Admission Form with accurate and complete details.
A photograph of the patient may be taken at the time of admission (for routine and stable cases).
You will be escorted to your assigned room by our Patient Care Assistant.
On Arrival At The Ward:
The nursing team at the respective nursing station will brief you about room amenities and record your meal preferences.
Patient will be provided with hospital attire.
Your vitals and medical history will be recorded by the nursing staff and resident doctor.
In case of surgery or procedure, informed consent will be obtained.
For any queries, please contact the ward nurse, head nurse, or Patient Relations Officer (PRO).

Note: All rooms are equipped with central oxygen & suction, an attendant bed and personal storage.
Hospital Diet & Meal Timings
Hospital diet is compulsory and can be availed based on your preferences. Food charges are not included in the treatment cost and will be billed separately. Only Vegetarian food will be provided.
